Job Summary
Provide direct, skilled nursing services to hospice patients in accordance with the Plan of Care under the direction of the RN Case Manager. Deliver compassionate, patient- and family-centered care to enable patients to spend meaningful time with loved ones, educate caregivers on home care, collaborate with the care team to deliver comfort and dignity, and support the patient and family throughout the hospice journey.
