Hospice Licensed Practical Nurse

Provide direct, skilled nursing services in accordance with the Plan of Care under the direction of the RN Case Manager. Build trusting, compassionate relationships with patients and their families while empowering caregivers through education on loved ones' care needs. Collaborate with the care team to deliver comfort, peace, and dignity, enabling patients to spend quality time with loved ones and engaging in activities they enjoy.
