Hospice -Licensed Practical Nurse

Job Summary
Hospice LPN covering Jackson, full-time. Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician’s orders, assessment data, and established standards. Delivers nursing visits, documents skilled nursing notes, communicates significant findings to the RN Case Manager and other personnel, reports unsafe conditions, implements the plan for patient safety, assists patients with self-care, suggests revisions to the plan of care, and provides services requiring substantial nursing skills under RN/physician direction. Collaborates with the RN Case Manager during normal business hours for guidance, and documents medication regimens and updates the medication profile. Delivers compassionate care to patients and families and works to prevent avoidable hospitalizations/ER visits. Must be graduate of an accredited nursing program, hold an active LVN/LPN license, have at least 1 year clinical experience (hospice/home health preferred), and be able to travel up to 50% with a valid driver’s license. Required Qualifications
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver’s license and insurance and reliable transportation to perform job tasks
