Job Summary
Hospice LMSW PRN role in Tomball/Spring TX providing social work visits to hospice patients in assisted living, independent living communities, skilled nursing facilities, and private residences within the Houston area. Responsibilities include conducting comprehensive assessments of patients' social and emotional needs, coordinating care and access to community resources, maintaining detailed documentation, utilizing EMR (Homecare Homebase) when available, developing personalized care plans with the interdisciplinary team, and providing counseling and support to patients and families facing end-of-life challenges. Requires LMSW licensure and minimum hospice and social work experience; payrate is $55-$75 per visit.
Required Qualifications
- Licensed LMSW
- Minimum of one year of hospice care experience
- Minimum of one year of social work experience
- Compassionate and empathetic approach to working with the elderly
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
