Job Summary
The Hospice Liaison facilitates patient discharge from facility to certified hospice agency, evaluates eligibility and resources, collects insurance information and authorizations, completes a comprehensive hospice care plan with patient/caregivers, coordinates services, documents in patient records, collaborates with care coordinators, social workers, and physicians, participates in committees, represents RRH Hospice Agency in community groups, and supports education and marketing activities.
Required Qualifications
- Registered Nurse: Associate’s degree in nursing
- Social Worker: Bachelor's degree in social work
