Hospice Liaison

Job Summary
Hospice Liaison responsible for developing referral relationships and driving census growth throughout Port Orange and Volusia County. Build and maintain partnerships with skilled nursing facilities, assisted living communities, physician practices, and other referral sources to generate qualified hospice referrals and admissions. Conduct regular face-to-face sales calls and community outreach, educate referral sources on hospice, palliative, and dementia care programs, identify new business opportunities, and collaborate with clinical and operational teams to ensure exceptional patient experience. Maintain accurate documentation of sales activities and referral pipeline; represent the organization at community events and industry meetings; consistently meet and exceed census and admission goals. Ideal candidate is a self-motivated, entrepreneurial sales professional who enjoys building territory and making a direct impact on organizational growth. Requires 2+ years healthcare sales experience and a valid driver’s license.
Required Qualifications
- Minimum of 2 years healthcare sales experience required
- Hospice, home health, post-acute care, physician services, wound care, or senior living sales experience strongly preferred
- Demonstrated success achieving and exceeding sales goals
- Strong relationship-building and business development skills
- Excellent communication and presentation abilities
- Self-starter with accountability and initiative
- Comfortable working independently in a developing market
- Valid driver's license and reliable transportation required
