Hospice Intake RN - Weekend [Part Time]

Triage calls from hospice patients and families to gather needs and symptom management information, providing nursing education based on care plans. Coordinate home care referrals over the phone from physician offices, hospitals, and community agencies, verifying insurance payors and documenting hospice focus, diagnosis, and demographics. Manage electronic medical records for referrals, name changes, and death processing, while scheduling field staff and arranging unscheduled or evaluation visits. Comply with regulatory standards, HIPAA requirements, and safe work practices, responding to emails and attending team meetings. Train new staff and participate in mentoring to support team maintenance of logs and statistics.
