Hospice Intake RN

Conduct hospice intake screening by receiving and coordinating incoming calls from referral sources, outside agencies, and field staff. Communicate with patients, clients, and families regarding care initiation arrangements while obtaining, documenting, and analyzing required information to make preliminary admission decisions. Log referrals, prepare reports, collect customer data into the management information system, and receive verbal physician orders where permitted by law. Provide telephonic contact and on-call support as needed. This full-time Monday through Friday office role is located in Grand Rapids, MI, with weekly pay ranging from $70k to $75k.
