Hospice Intake RN

Act as a telephone liaison interfacing with hospice referral sources, outside agencies, health professionals, and field staff. Receive and coordinate incoming calls, providing patients, clients, and referral sources with basic information to access appropriate services. Communicate with patients and families regarding care initiation arrangements, complete intake screenings by documenting required information to make preliminary admission decisions, and receive verbal orders from physicians where permitted. Log referrals, prepare reports, enter customer data into the management information system, and provide telephonic contact as needed. Work a full-time Monday through Friday in-office schedule with weekly pay ranging from $70k to $75k.
