Hospice Intake RN

Act as a contact telephone liaison interfacing with hospice referral sources, outside agencies, health professionals, and field staff. Receive and coordinate all incoming calls, providing patients, clients, and referral sources with basic information to assist in accessing appropriate services. Communicate with hospice patients, clients, and families regarding arrangements for the initiation of care or services. Complete intake screenings by obtaining, documenting, and analyzing required information to make preliminary admission decisions. Log referrals, prepare reports, respond to inquiries, and provide telephonic contact as needed. Receive verbal orders from physicians where permitted by law and collect customer information into the management information system. This role requires current Michigan RN licensure, one year of RN experience, and a full-time Monday through Friday office schedule in Jenison, MI.
