Hospice Intake RN

Coordinate incoming calls from hospice referral sources and field staff to facilitate patient admissions. Complete intake screenings by documenting required information and analyzing data to make preliminary admission decisions. Communicate with patients and families regarding care arrangements while receiving and logging referrals. Enter customer information into the management system and provide telephonic contact as needed. Act as a liaison for outside agencies and physicians where permitted by law. This full-time Monday through Friday office role in Allendale, MI offers weekly pay of $70-75k and includes health, dental, vision, and 401(k) benefits.
