Job Summary
Hospice Intake Coordinator responsible for referral coordination, documentation, and collaboration with the admissions team in a full-time, on-site role at Capstone Hospice in Peachtree Corners, GA. Duties include receiving and documenting patient referrals, entering information into EMR, coordinating evaluation/admission processes, communicating with referral sources and care teams, ensuring eligibility verification of benefits, scheduling evaluations, and educating patients, families, physicians, and hospitals about hospice services. Requires associate degree or equivalent, hospice experience, proficiency with EHR/EMR, and knowledge of Medicare/Medicaid and regulatory guidelines, along with strong communication, empathy, confidentiality, problem-solving, and the ability to work under stress in a fast-paced environment.
Required Qualifications
- Associate degree or equivalent experience
- Previous hospice experience in a direct hospice position
- Proficiency in electronic data management and EMR systems
- Knowledge of Medicare, Medicaid and third-party reimbursement requirements
- Knowledge of regulatory guidelines governing hospice admissions
- Exceptional verbal and written communication
- Empathy, integrity, professionalism with patient/family/referral sources
- Ability to manage sensitive and confidential information
- Strong attention to detail
- Critical thinking and problem-solving skills
- Ability to work independently with a variety of individuals
- Ability to handle high levels of stress
- Flexibility and adaptability in a fast-paced environment
