Job Summary
Coordinate patient and family care in collaboration with the primary physician and interdisciplinary team. Conduct thorough assessments of patient and family needs and ensure timely delivery of interventions outlined in the care plan. Monitor patient conditions, provide clinical interventions, and deliver education to enhance comfort and quality of life. Address concerns promptly to maintain high satisfaction and trust. Work nights from 7pm to 7:30am, two days per week, with weekend availability. Requires Florida RN license, BLS certification, and one year of relevant nursing experience.
Required Qualifications
- Registered Nurse currently licensed in the State of Florida
- Current Basic Life Support Certification
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license
- 2 days 7pm - 7:30am
- weekend availability
Desired Qualifications
- One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
- Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus
