Job Summary
Provide direct patient care in accordance with the hospice plan of care, administering treatments, medications, and diet as tolerated. Collaborate with interdisciplinary team members including social workers, chaplains, and physicians to address physical, psychosocial, and spiritual needs. Establish measurable patient goals, communicate the plan of care, and adjust priorities based on immediate clinical status. Conduct patient and caregiver teaching, document understanding, and incorporate learning needs into discharge plans. Complete all electronic documentation at the point of care and assist field teams when needed. Requires current Maryland RN license, two years of med/surg or home care experience, and valid driver's license. Schedule includes nights from 7pm to 7:30am.
Required Qualifications
- Current Maryland RN license or accepted reciprocal out-of-state nursing licensure
- 2 or more years of nursing experience in med/surg or home care
- Valid driver's license with an acceptable driving record and reliable transportation
- Ability to meet the physical demands of the position with or without reasonable accommodation
- Vaccination against influenza
- Screening for TB upon hire and annually thereafter
Desired Qualifications
- CHPN certification
- Experience in hospice, end-of-life, or critical care
- Strong and creative, problem-solving skills with the ability to prioritize and delegate
- Familiarity and comfort with documenting in an Electronic Medical Record
