HOSPICE INPATIENT ADMISSIONS NURSE

Job Summary
Perform comprehensive hospice admission assessments in hospitals, hospice inpatient units, and contracted inpatient facilities; serve as a clinical liaison to referral sources, patients, families, and facility partners; develop plans of care for patients admitted with physician approval; coordinate patient care across disciplines; document patient care data; evaluate patients for hospice admission using comprehensive physical assessments and psychosocial/environmental considerations; update the plan of care in coordination with the Clinical Manager; make referrals to appropriate disciplines and resources; travel as required; include other duties as assigned.
Required Qualifications
- Current RN licensure within the state of the branch location (North Carolina)
- At least two years of home health clinical nursing experience
- Successfully completed nursing education from an approved school of nursing
- Must accept travel and working conditions as assigned
- Must be able to work independently and possess strong communication, organization and time management skills
- Must be able to use a computer and have a working knowledge of computer applications (e.g., Windows, Word)
- CPR certification is required
- Minimum of twelve (12) hours continuing education annually
- Must have valid driver's license
