HOSPICE INPATIENT ADMISSIONS NURSE

Job Summary
Conduct comprehensive hospice admission assessments in hospitals, inpatient units, and contracted facilities, evaluating patients for physician orders by reviewing physical systems and identifying psychosocial and environmental factors. Develop and update plans of care with certifying physician approval, coordinate patient services across disciplines and Clinical Managers, and document care data accurately. Make referrals to appropriate resources as ordered while serving as a clinical liaison to referral sources, patients, families, and facility partners. Requires current RN licensure, two years of home health experience, CPR certification, and valid driver's license. Full-time role with travel and working conditions as assigned.
Required Qualifications
- current RN licensure within state of branch location
- at least two years of home health clinical nursing experience
- Successfully completed nursing education from an approved school of nursing
- Must accept travel and working conditions as assigned
- Must be able to work independently
- strong communication skills
- organization skills
- time management skills
- working knowledge of a variety of computer applications such as Windows, Word, etc.
- CPR certification
- Minimum of twelve (12) hours continuing education annually
- valid driver's license
- Background checks/drug-free workplace
