Job Summary
Assist patients with activities of daily living including ambulation, toileting, bathing, dressing, oral care, and eating. Perform light homemaker assistance such as changing linens, tidying rooms, preparing simple meals, and delivering supplies. Obtain vital signs, follow the plan of care, and document changes in condition or unusual incidents in the electronic health record. Communicate with families, reinforce education, and participate in service planning meetings. Precept new staff and delegate tasks to volunteers. Requires Certified Nursing Assistant certification, one year of personal care experience, and a valid driver's license. Full-time 0.7 FTE day shift with weekend and holiday rotations.
Required Qualifications
- High school diploma or GED, or equivalent combination of education and work experience
- Graduation from an accredited Certified Nursing Assistant program
- Current, unrestricted Nursing Assistant Certification in state of employment
- Valid driver's license, auto insurance, and access to a reliable vehicle
- Basic Microsoft Office skills
- Experience using electronic health records
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions
Desired Qualifications
- One-year personal care experience, hospice, or home health, preferred
