Job Summary
Conduct hospice homecare case management for patients in western North Carolina, coordinating care across the full continuum of post-acute services including rehabilitation, home health, adult care, and palliative care. Obtain Basic Cardiac Life Support, Registered Nurse certification, and a driver's license within 30 days of employment start date. This role supports CarePartners Health Services, a member of Mission Health and HCA Healthcare, dedicated to providing compassionate post-acute care to help people of western North Carolina live full and productive lives despite illness, injury, or aging-related issues.
