Job Summary
The Hospice Home Team Weekend RN delivers compassionate, patient-centered hospice care on weekend shifts (8am-8pm, every other Saturday and Sunday) across territories including Sheboygan, Manitowoc, and potentially Ozaukee Counties. Responsibilities include conducting comprehensive patient assessments, administering prescribed medications and treatments, collaborating with physicians, social workers, and chaplains to develop and update individualized care plans, providing education and emotional support to patients and families, documenting interactions in the electronic health record, and coordinating with on-call providers as needed to ensure timely response to changes in patient conditions. The role emphasizes holistic care, patient dignity, and adherence to hospice care protocols and regulatory requirements. Skills emphasize clinical nursing proficiency, strong communication, critical thinking, organizational skills, and effective documentation to support continuity of care across weekend shifts.
Required Qualifications
- Current and valid RN license in state of practice (WI)
- Minimum of two years clinical nursing experience (hospice/palliative/home care preferred)
- BLS certification
- Ability to perform comprehensive patient assessments and develop care plans
- CHPN certification within 3 years of hire
