Job Summary
Provide skilled nursing care to hospice patients in their homes while working independently in the field as part of a supportive interdisciplinary team. Maintain electronic charting and manage case management duties for the assigned roster. This full-time position requires an active New York Registered Nurse License with at least two years of patient care experience. Candidates must possess a valid driver's license and reliable transportation, working Monday through Friday from 8:30 AM to 4:30 PM. Authorized to work in the United States; no employer-sponsored visas available.
