Hospice/Home Health Community Liaison

Job Summary
Build and maintain relationships with healthcare providers, hospitals, and community organizations to support patient access to hospice care. Identify and secure new referral sources through targeted education and outreach efforts while representing Blize Healthcare at trade shows and professional gatherings. Educate the community about hospice and Home Health services and monitor referral data to identify trends. Collaborate with clinical teams to ensure seamless patient transitions. Requires strong communication skills, sales experience, and knowledge of the care continuum. Full-time role in Santa Clara County with a competitive bonus structure and comprehensive benefits.
Required Qualifications
- Strong communication, interpersonal, and presentation skills
- Proven ability to build and maintain professional relationships
- Sales or marketing experience, preferably in the healthcare industry
- Knowledge of hospice and Home Health care continuum, services, and regulatory requirements
- Associate's degree or equivalent experience
- Ability to lift to 50 pounds as needed
- Reliable transportation for travel within the assigned territory
