Hospice GIP Liaison/RN - Lower Fairfield County

Provide skilled nursing care, administer medications, and coordinate comprehensive plans of care for patients in home settings across Greenwich, Stamford, and Norwalk hospitals. Perform physical and emotional assessments, report status changes to supervisors and physicians, and participate in weekly interdisciplinary team meetings. Maintain compliance documentation through computer-based systems, submit clinical records and time logs, and evaluate home health aides as needed. Requires RN licensure, one to two years of experience, and previous hospice care. Weekend and holiday coverage is required.
