Hospice GIP Admission Nurse (RN)

Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs, utilizing the nursing process to write and initiate plans of care. Administer prescribed medications and treatments while counseling patients and caregivers on self-care techniques, diet, and medication instructions. Coordinate patient care by communicating with physicians and community health resources, ensuring continuity through accurate clinical documentation and equipment arrangements. Act as Case Manager when assigned to oversee caseloads and participate in quality improvement teams. Work Sunday through Wednesday.
