Hospice Float Case Manager

Manage a case load of 12-14 patients, averaging four daily, while providing float coverage across Atlanta and Roswell territories. Serve as the back-up call resource for the hospice team. Maintain clinical excellence and quality care standards for each member of the family. This registered nurse role requires one year of hospice or palliative care experience and two years of in-home patient care experience. Familiarity with the KanTime EMR system is preferred. The position offers an annual salary of $85,000-$93,000, comprehensive health benefits including employer-paid premiums, and a 401(k) with a 3% match.
