Job Summary

Hospice Field Team Manager (RN) leads an interdisciplinary team of nurses, home health aides, social workers, and chaplains delivering end-of-life care in the territory north of I-10. Responsibilities include supervising staff, coordinating admissions and Plan of Care updates, ensuring regulatory compliance and budget adherence, conducting IDT meetings, providing staff education, maintaining CEU credits, and fostering strong relationships with patients, families, and facilities to achieve high-quality, patient-centered outcomes. Requires RN license in Texas, a Bachelor’s degree in nursing preferred, 5+ years of management experience in hospice/palliative settings, travel across multiple locations, and a valid Texas driver’s license with liability insurance; CPR certification is required.

Required Qualifications

  • Graduate of an accredited school of nursing with current RN license to practice in Texas
  • Bachelor’s degree in nursing preferred
  • Home health, hospice or public health nursing experience
  • Ability to supervise staff and coordinate services
  • Five years minimum management experience, hospice/palliative experience preferred
  • Excellent organizational, administrative, interpersonal and communication skills
  • Ability to travel to multi-work locations during workday
  • Must have reliable vehicle, valid Texas Driver’s License and Liability Insurance
  • Current CPR certification
