Job Summary
Provide terminal-patient hospice nursing care in home and other care settings. Conduct comprehensive assessments of physical, functional, psychosocial, and environmental needs; develop and document plans of care; educate patients and families on disease processes, self-care, end-of-life care, and nutritional needs; coordinate with the interdisciplinary group to ensure quality and continuity of care; supervise Hospice Aide and LPN/LVN; determine scope and frequency of services based on acuity; maintain clinical progress notes and communicate changes to the physician; perform on-call duties as assigned.
Required Qualifications
- Current RN License
- CPR Certification
- Valid Driver’s License
- Valid Auto Insurance
- At least 2 years of Hospice RN experience preferred
- HomeCare HomeBase experience preferred
