Job Summary
Conduct physical and psychosocial assessments for MJHS Hospice and Palliative Care patients in both community and facility settings, then electronically document visit occurrences. Develop and implement nursing plans of care while making recommendations based on individual patient needs. This role requires working one weekend per month and functioning in a backup On Call capacity 2-3 times per month, with car and escort service available for shifts. Candidates must hold an NYS RN license and a driver's license, with an associate's degree or BSN preferred.

- Associates/Diploma from an accredited school of Nursing
- BSN
- 2+ years' experience in a med/surg, community health, and/or oncology nursing setting
