Job Summary
Conduct physical and psychosocial assessments for hospice patients in community and facility settings while electronically documenting visit occurrences. Develop nursing plans of care and recommend interventions based on patient needs. Work one weekend per month and function in a backup on-call capacity 2-3 times monthly, with car and escort service provided for shifts. Requires an NYS RN license, NYS driver's license, and 2+ years of experience in med/surg, community health, or oncology nursing.
