Job Summary
Conduct physical and psychosocial assessments of MJHS Hospice and Palliative Care patients in community and facility settings, then electronically document visit occurrences. Develop and implement nursing plans of care while making recommendations based on individual patient needs. This role requires one weekend per month and backup on-call capacity of 2-3 times monthly, with car and escort service available for shifts. Candidates must hold a New York State RN license and a driver's license, with an Associate's or BSN degree and 2+ years of experience in med/surg, community health, or oncology settings preferred.
Required Qualifications
- Associates/Diploma from an accredited school of Nursing
- NYS RN license
- NYS Driver's license
