Job Summary
Conduct physical and psychosocial assessments for MJHS Hospice and Palliative Care patients in community and facility settings while electronically documenting visit occurrences. Develop and implement nursing plans of care, making recommendations based on patient needs. Work one weekend per month and function in a backup on-call capacity 2-3 times monthly, with car and escort service available for shifts. Requires an RN license and 2+ years of experience in med/surg, community health, or oncology. We deliver patient-centered, equitable care with compassion and respect.
Required Qualifications
- NYS RN license
- NYS Driver's license
Desired Qualifications
- Associates/Diploma from an accredited school of Nursing
- BSN
- 2+ years' experience in a med/surg, community health, and/or oncology nursing setting
