Job Summary
Conduct physical and psychosocial assessments of MJHS Hospice and Palliative Care patients in community and facility settings, then electronically document visit occurrences. Develop and implement nursing plans of care while making recommendations based on patient needs. Work one weekend per month and function in a backup on-call capacity of 2-3 times monthly, with car and escort service available for shifts. Requires an RN license, driver's license, and 2+ years of experience in med/surg, community health, or oncology settings.
Required Qualifications
- Associates/Diploma from an accredited school of Nursing
- NYS RN license
- NYS Driver's license
Desired Qualifications
- BSN
- 2+ years' experience in a med/surg, community health, and/or oncology nursing setting
