Job Summary
Conduct physical and psychosocial assessments for MJHS Hospice and Palliative Care patients in both community and facility settings, then electronically document visit occurrences. Develop and implement nursing plans of care while making recommendations based on patient needs. Work a schedule including one weekend per month and backup on-call capacity 2-3 times monthly, with car and escort service available for shifts. Requires an NYS RN license, driver's license, and 2+ years of experience in med/surg, community health, or oncology nursing. The role supports a mission of delivering equitable, compassionate, patient-centered care.
