Job Summary
Hospice LPN – Crisis Care (Night shift) providing ongoing monitoring of physical, functional, psychological, social, spiritual, and safety needs of patients/families, administer medications and treatments per plan of care, deliver skilled nursing care under the direction of the Primary RN, and collaborate with the Crisis Care Team. Responsibilities include educating the community about Empath Health philosophy, representing the organization at nursing functions, and supporting patients and families through end-of-life care in homes and facilities within Marion County. The role requires a valid Florida LPN license, CPR certification, and a Florida driver’s license, with IV therapy certification preferred and the ability to travel regionally as needed. Required Qualifications
- Valid Florida LPN License
- Valid Florida State Driver’s License
- Valid CPR certification from the American Red Cross or American Heart Association
- Current IV Therapy certification preferred (must obtain within six months of hire)
- Reliable transportation to perform tasks/tasks in a timely fashion
- Experience in healthcare settings desirable
