Job Summary
Conduct initial physical and psychosocial assessments of potential hospice patients to develop individualized plans of care, goals, and recommendations based on specific needs. Admit eligible patients onto the Hospice program following National Hospice and Palliative Care Organization standards while performing evaluations in private homes, hospitals, or nursing facilities. Requires NYS RN license and 2+ years' experience in med/surg, community health, or oncology settings. Offers $15,000 sign-on bonus or student loan assistance.
Required Qualifications
- Associates/Diploma from an accredited school of Nursing
- NYS RN license
- NYS Driver's license
Desired Qualifications
- BSN
- 2+ years' experience in a med/surg, community health, and/or oncology nursing setting
