Job Summary
Hospice Crisis Care LPN PT days responsibilities include providing 1:1 bedside care to actively declining patients in homes or facilities under RN supervision, driving to patient locations within the Dayton/Springfield area, delivering compassionate hospice care, documenting services, and maintaining CPR certification and valid license. The role requires driving capability, a valid Ohio LPN license, and ability to pass background and drug screenings; offers competitive salary and benefits with mileage reimbursement and company devices.
Required Qualifications
- Registered LPN with the State of Ohio without any board actions.
- One year of nursing experience preferred.
- CPR Certified.
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime or inclement weather.
- Valid Driver's License with Safe Driving Record.
- State Minimum Automobile Insurance Coverage.
- Must be able to pass a criminal background check
- Must be able to pass a 10 panel drug screen
