Job Summary
8pm-8am full-time Hospice Crisis Care LPN providing 1:1 bedside care for patients actively declining in homes/facilities within the Dayton, OH area. Works under RN supervision, driving to patient locations as needed; responsibilities include direct patient care, documentation, and ensuring compassionate support to patients and families in crisis. Mileage reimbursement and technology provided; competitive salary and comprehensive benefits. Must be a licensed Practical Nurse in Ohio with CPR certification and a valid driver’s license; ability to drive in various weather and pass background and drug screening. Must have no board actions on Ohio license and basic computer skills for charting.
Required Qualifications
- Registered LPN with the State of Ohio without any board actions
- One year of nursing experience preferred
- CPR Certification
- Computer skills sufficient to properly document services and care
- Ability to drive during daytime, nighttime or inclement weather
- Valid Driver's License with Safe Driving Record
- State Minimum Automobile Insurance Coverage
- Must be able to pass a criminal background check
- Must be able to pass a 10 panel drug screen
