Job Summary
Hospice Crisis Care LPN - Full-time day shift (8am-8pm) serving Dayton area patients with 1:1 bedside care in homes or facilities under RN supervision. Role requires driving to patients, delivering comfort and care to those actively declining, with responsibilities including accurate documentation and coordination of care using a computer system. Must hold an Ohio LPN license (no board actions) and CPR certification; valid driver’s license with safe driving record; meet background and drug screening requirements. Required Qualifications
- LPN license in the State of Ohio
- No board actions on Ohio LPN license
- CPR certification
- Valid driver’s license with safe driving record
- Auto insurance coverage as required by state law
- Criminal background check
- 10-panel drug screen
- Ability to drive during daytime, nighttime or inclement weather
- Computer skills sufficient to document services and care
