Job Summary
Assist clients with Activities of Daily Living including bathing, medication reminders, and vitals while documenting medical condition changes for the nurse case manager. Provide shift reports to ensure consistent care delivery and represent the Providence mission on duty. Travel to South Holland and surrounding areas to deliver individualized personal care in accordance with the hospice aide plan of care. Mileage is reimbursed at $0.76/mile with an hourly rate of $18.80 to $21.00 for a part-time schedule of 10-15 hours/week.
Required Qualifications
- Must be a CNA in the State of Illinois
- Must have a compassionate, empathetic and nurturing personality
- Comfortable traveling throughout the organizational geographic region
Desired Qualifications
- Home health and/or hospice experience
