Job Summary
Hospice CNA provides personal care under the supervision of a nurse according to the patient's plan of care in home care settings. Responsibilities include assisting with ADLs, safely ambulating/transferring patients, providing wound/skin/bowel care as directed, performing passive/active ROM exercises, recording care and observations, participating in care conferences and team meetings, completing charting and required documentation, and supporting caregiver education. Flexible with variable shifts and weekends; participates in on-call rotation; collaborates with transdisciplinary team to ensure patient-centered care and quality improvement activities. Requires Florida state-tested CNA certification and 12 months of CNA experience; Florida CNA license and background screening are required.
Required Qualifications
- Must be state tested nursing assistant in good standing and have 12 months of experience
- Must be a Florida Certified Nursing Aide-CNA
- Background screening through the Care Provider Background Screening Clearinghouse
