Job Summary
Provide direct hospice patient care across home settings (residences, assisted living, SNFs) as a CNA. Deliver bathing, peri-care, dressing, mouth care, toileting, incontinence care, repositioning, and meal assistance. Document at bedside in EMR, use company cellphone, and coordinate with RN case managers. Travel between patients with mileage reimbursed; maintain vehicle supplies and pick up new supplies at the local branch. Train caregivers to provide personal care within scope, coordinate visit times with patients/caregivers, and ensure timely visits. Typical schedule is Monday–Friday, 8 AM–5 PM after orientation; most CNAs see 5–7 patients per day with 45–60 minutes with each patient. Location listed includes Jacksonville, NC, US; travel between patient visits and collaboration with nursing staff are core duties.
Required Qualifications
- High school diploma or equivalent
- Completion of CNA course
- Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina/South Carolina
