Job Summary
Provide direct personal care under RN supervision as a field-based CNA in patients' homes or care settings, following the Plan of Care and patient preferences. Assist with mobility and safety, observe and report changes to the RN, and maintain infection control, privacy, and dignity. Requires state-approved CNA training, active Texas CNA certification, and a high school diploma or equivalent; at least one year of hospice/home health experience is preferred. Strong communication, ability to read/write in English, and reliable transportation with a valid Texas driver’s license are required. Work locations include private residences, assisted living, nursing facilities, or memory care settings; duties span indoors and outdoors with exposure to infectious diseases and bodily fluids.
Required Qualifications
- State-approved CNA training program
- Active CNA certification in the State of Texas
- High school diploma or equivalent
- At least one year of experience in hospice, home health, or similar care setting preferred
