Job Summary
Provide personal care with dignity, monitor vital signs, and promptly report patient changes to the interdisciplinary care team. Communicate ethically with patients and families while maintaining thorough documentation in line with organizational standards. Travel locally across Springfield, Cass, Morgan, Mason, Sangamon, and surrounding areas to deliver unhurried, dignified support in patient homes. Maintain a calm, dependable presence under emotionally demanding circumstances while attending to physical, emotional, and spiritual needs.
Required Qualifications
- Active CNA License or Certification in the state of employment
- Possess and maintains current CPR certification
- A valid driver's license
- Willingness to travel locally for patient visits
Desired Qualifications
- One (1) + year Hospice, home health or related organization experience
