Hospice CNA Albuquerque

Provide personal care and emotional support to hospice patients in Skilled Nursing Facilities, assisted living, and private residences. Assist with activities of daily living including bathing, grooming, toileting, and transfers. Observe and report changes in patient condition to the clinical team while maintaining accurate documentation per regulatory guidelines. Support families with education and comfort measures and collaborate with nurses, social workers, and chaplains. Travel throughout the Albuquerque service area daily. Requires current New Mexico CNA license, valid driver's license, and ability to work independently within a supportive team culture.
