Job Summary
Lead the Patient Care Team by supervising, evaluating, and coordinating interdisciplinary members to assure continuity of care from admission through discharge or transfer to bereavement. Direct all patient care services including volunteer and bereavement components, while reviewing pending admissions and discharges to ensure eligibility and appropriateness criteria are met. Collaborate with physicians on palliative measures, manage 24/7 team schedules, and facilitate interdisciplinary meetings to revise care plans. Oversee staff interviews, training, and performance through field visits, while approving payroll and monitoring resource utilization for cost containment. Monitor clinical records for regulatory compliance, conduct chart reviews, and address customer grievances to prevent unnecessary revocations. Participate in on-call rotation, quality improvement activities, and annual program reviews.
Required Qualifications
- Reliable transportation with appropriate license and insurance coverage for driver and passengers
- Private telephone in home
- Current and valid R.N. License to practice in the state where the VITAS program is located
- Knowledge of the principles and practices of primary medical and nursing care
- At least a working knowledge of oncologic nursing and palliative care
- Basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary health care team
- Knowledge of the process of dying and bereavement
- Working knowledge of VITAS information system (Vx) and the ability to utilize management reports
- Two years successful supervisory experience or equivalent in a health care organization
