Job Summary
Clinical Nurse II in Hospice/Home Care providing care for a group of complex patients in the home setting, with responsibilities including initial assessments, ongoing monitoring, care planning and implementation, recording observations in the PointCare EMR, and coordinating nursing care with an interdisciplinary team. Role includes precepting new clinical staff, educating patients and caregivers on disease processes, symptom control, and wound care. Requires graduation from a state-accredited nursing program and current Ohio RN licensure; ACLS within 6 months of ER assignment and ongoing BLS/CPR certification; 12 annual CE hours including Fire Safety and BLS/CPR recertification. Emphasizes compassionate, ethical nursing practice, alignment with Compassus values, and delivering home-based care across the hospice continuum, with opportunities for leadership pathways and professional development.
Required Qualifications
- Graduation from a state accredited school of nursing
- Current licensure from the state of Ohio as an RN
- ACLS certification within 6 months of assignment to the ER
- BLS/CPR certification
- 12 contact hours of continuing education annually including Fire Safety and BLS/CPR Recertification
