Hospice Clinical Manager - RN

Job Summary
Hospice Clinical Manager - RN leads visits from referral to admission through death, ensures quality care, documentation, and collaboration within guidelines, coordinates hospice eligibility and recordkeeping, provides orientation, education, training, and competencies, and maintains/implements operational policies to meet patient and family needs in compliance with federal, state, and local requirements. Supports administrative directives and leadership. Requires active RN license and CPR; hospice or palliative care experience; leadership/charge nurse experience; two years of recent hospice/home health/public health nursing experience (hospice preferred); CHPN preferred or obtainable within 24 months. Required Qualifications
- Active RN license
- Active CPR certification
- Hospice or palliative care experience
- At least one year in a leadership or charge nurse position or related experience
- Two years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred
- CHPN certification preferred or ability to obtain within 24 months of employment
