Job Summary
Oversee all visits from referral-to-admission through death processes, ensuring quality care, documentation, and collaboration meet federal, state, and local guidelines. Support direct care coordination, verify hospice eligibility, and manage recordkeeping functions while maintaining operational policies for patient and family needs. Provide orientation, education, training, and competency development for the clinical team. This role requires an active RN license, CPR certification, and at least one year in a leadership or charge nurse position, with two years of recent clinical experience in hospice, oncology, home health, or public health nursing. Preferred qualifications include a Hospice and Palliative Registered Nurse Certification (CHPN). Required Qualifications
- Active RN license (or ability to activate) in the state(s) in which you operate
- Active CPR certification
- Hospice or palliative care experience
- At least one year in a leadership or charge nurse position or relative experience
- Two years of recent clinical experience in hospice, oncology, home health or public health nursing
