Hospice Clinical Liaison-RN required

Job Summary
Assist referral sources in identifying patients with care needs matching hospice services and explain services to patients, families, and significant others. Obtain signatures on required forms, ensure all medical records are turned in to the Admission Coordinator, and facilitate the admission process to and from hospital and long-term care-based referral sources. Assess referred patient's clinical needs and disease status, confer with attending and hospice physicians to establish eligibility, and facilitate patient discharge by coordinating transportation, DME, medication needs, supplies, and admission by a home hospice nurse. Oversees GIP patient admission process based on identified symptom management needs and works with facilities to identify, transition, and admit patients appropriately for general inpatient services.
Required Qualifications
- RN License
- Bachelor's degree in Nursing
- Two (2) to three (3) years of related experience
- Hospital and/or long-term care clinical experience
- Experience in healthcare marketing
- Active and unencumbered Registered Nurse license in state(s) of employment
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Routine use of standard medical equipment and tools associated with clinical care
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Articulates and embraces hospice philosophy
