Hospice Clinical Liaison - RN required

Job Summary
Model Compassus values and hospice philosophy by assisting referral sources in identifying patients matching provided services. Explain hospice services and benefits to patients, family members, and significant others while obtaining and processing required signatures and medical records. Facilitate the admission and discharge process by assessing clinical needs, conferring with physicians, and coordinating transportation, DME, and medication supplies. Establish professional relationships with hospitals and facilities to meet growth targets and maintain market acceptance. Communicate identified needs and potential solutions to the Supervisor and attend staff meetings, educational programs, and community events as requested.
Required Qualifications
- RN licensure
- Active and unencumbered Registered Nurse license in state(s) of employment
- Two (2) to three (3) years of related experience
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
Desired Qualifications
- Bachelor's degree in Nursing
- Hospital and/or long-term care clinical experience
- Experience in healthcare marketing
