Job Summary
Hospice Clinical Liaison RN responsible for hospital-based referrals, hospice eligibility assessments, and coordinating transitions from hospital to hospice. Works with physicians, case managers, social workers, and discharge planners to ensure timely admissions and appropriate levels of care. Travel across the south side of Chicago and surrounding south suburbs to partner with hospitals and healthcare systems. Schedule Monday–Friday with no nights and one weekend every 6-8 weeks. Training provided for nurses transitioning from acute care to hospice liaison roles. Requires Illinois RN license, 1-2+ years of RN experience, CPR certification, and a valid driver's license.
Required Qualifications
- Active Illinois RN license
- Graduate of an accredited nursing program
- 1-2+ years of RN experience (hospital or hospice experience preferred)
- Strong communication and interpersonal skills
- Current CPR certification
- Valid driver’s license and insurance
